- Your passport (unexpired, and valid for six months beyond your date of intended entry to U.S.)
- Two (2) color photographs of each person applying for a visa (5 cm x 5 cm, or 2 inch x 2 inch).
- DS-260 Confirmation Page
- Confirmation page from your online selection of a delivery address to return your passport
- Original and copy of civil documents, including your birth certificate, and if applicable, your marriage certificate (if applicable), divorce decress, and death certificates.
Certificates issued in Somalia will not be accepted. - Medical examination results in sealed envelope
- Original and certified copies of birth certificates for all children of main applicant, even if the child is not immigrating to U.S.
- I-864 Affidavit of Support and supporting documents (if your application is based on family relationship)
