  • Your passport (unexpired, and valid for six months beyond your date of intended entry to U.S.)
  • Two (2) color passport-size photographs of each person applying for a visa (5 cm x 5 cm, or 2 inch x 2 inch).

  • Original and copy of civil documents, including your birth certificate, and if applicable, your marriage certificate (if applicable), divorce decress, and death certificates.
  • Medical examination results in sealed envelope
  • Original and certified copies of birth certificates for all children of main applicant, even if the child is not immigrating to U.S.

  • I-864 Affidavit of Support and supporting documents (if your application is based on family relationship)
  •  The joint sponsor may submit a personal letter explaining their relationship with the applicant and how they intend to support the applicant after they arrive in the United States.
  • Proof of U.S. Petitioner’s legal status and domicile in the United States (if your application is based on family relationship)
