  • Your passport (unexpired, and valid for six months beyond your date of intended entry to U.S.)
  • Two (2) color passport-size photographs of each person applying for a visa
  • DS-260 Confirmation Page
  • Fee or Money receipt of the medical examination as a proof that you have appeared for the medical examination
  • I-864 Affidavit of Support and supporting documents (if your application is based on family relationship)
  • A photocopy of the sponsor’s IRS transcript, as well as any relevant W-2s
  • Proof of U.S. Petitioner’s legal status and domicile in the United States (if your application is based on family relationship)
  • Evidence of relationship between petitioner and visa applicant (if your application is based on family relationship)
  • Original civil and religious marriage certificates, notarized English translations, and color photocopies
  • Original Police clearance certificate issued within a year for all applicants 16 years of age or older
  • Letter from your U.S. employer dated less than one month ago.
  • Original and copy of criminal and court records, if you have been arrested
  • Original and copy of military records
  • Original and copy of adoption documents
  • Original and certified copies of birth certificates for all children of main applicant, even if the child is not immigrating to U.S.
