- Two (2) color passport-size photographs of each person applying for a visa
- Two (2) full face color visa photos for each applicant taken within 6 months
- Identification (ID) Card and Household Registration Book: Copy and original
- Your passport (unexpired, and valid for six months beyond your date of intended entry to U.S.)
- Original and certified copies of birth certificates for all children of main applicant, even if the child is not immigrating to U.S.
- Medical examination results in sealed envelope
- Original Vietnam’s Justice Record Check #2 and original police certificate from any foreign country in which you resided for at least twelve months since age of 16 or older
- I-864 Affidavit of Support and supporting documents (if your application is based on family relationship)
