- Your passport (unexpired, and valid for six months beyond your date of intended entry to U.S.)
- DS-260 Confirmation Page
- Original and copy of civil documents, including your birth certificate, and if applicable, your marriage certificate (if applicable), divorce decress, and death certificates.
- Medical examination results in sealed envelope
- Original and certified copies of birth certificates for all children of main applicant, even if the child is not immigrating to U.S.
